Halton Haven Hospice offers patient centred Palliative and End of Life Care for anyone with a life limiting illness that has been identified as being in the last year of their life. In line with the North West End of Life Care Model, the Hospice aims to support the individual, their family and carers through this difficult period. As a community based resource the Hospice is constantly exploring ways in which to make the transition between Hospital, Community and Hospice as seamless as possible.
Dr Averil Fountain Community Consultant in Palliative Care is working with the Hospice supporting the development of services with the aim of being recognised as the Specialist Palliative Care Provide for Halton.
The Hospice no longer has Continuing Care beds and we are unable to accept patients for long term care or pre booked respite. The average length of stay for a patient is just under two weeks. However, some patients with complex needs may require a longer stay. We coordinate appropriate discharge planning through liaison with multi agency services.
